Home Modification Assessments

NDIS Home Modification Assessments

Our NDIS Home Modification Assessments help identify and address environmental barriers so that individuals can live safely, independently, and comfortably in their own homes. Conducted by experienced occupational therapists, these assessments ensure that any recommended modifications align with the NDIS’s ‘reasonable and necessary’ guidelines.

Why You Might Need This Assessment:

  • Reduced Risk of Falls and Injuries: Uneven surfaces, narrow doorways, or steep steps can lead to falls. Assessments pinpoint hazards and recommend solutions.
  • Changes in Mobility or Strength: Following surgery, injury, or progression of a disability, your mobility needs may change. Home modifications can match these evolving needs.
  • Managing Carer Capacity: When family members or support staff assist with personal care or transfers, modifications can decrease the physical strain and risk of injury for everyone.
  • Preparing for Long‑Term Ageing in Place: Planning ahead with grab rails, accessible switches, and bathroom adaptations can help you stay in your home as your needs change over time.
  • Supporting Equipment Use: If you require mobility aids (e.g. wheelchairs, walkers) or specialised equipment (e.g. ceiling hoists), modifications ensure these can be used safely and effectively.

What’s Included in the Assessment:

  • Comprehensive Home Visit: Walkthrough of entry points, living areas, kitchen, bathroom, bedroom, and outdoor spaces.
  • Functional Task Analysis: Observation of daily activities—such as transfers, showering, and cooking—to identify specific challenges.
  • Collaborative Goal‑Setting: Discussion with you, your family, and support team to prioritise modifications that best meet your goals.
  • Detailed Written Report: Clear recommendations with technical specifications, cost estimates, and implementation priorities.
  • Support with Quotes and Builders: Guidance on obtaining competitive quotes and liaising with trades to ensure accurate implementation.
  • Follow‑Up Review: Optional post‑modification visit to confirm effectiveness and make any necessary adjustments.

Outcomes and Benefits:

  • Increased independence and confidence in daily tasks.
  • Improved safety and reduced likelihood of falls or injuries.
  • Enhanced quality of life and ability to remain in familiar surroundings.
  • Better use of specialist equipment and reduced carer strain.
  • Assurance that modifications meet NDIS funding criteria.
